Located in the heart of Midwood, Brooklyn Connected Care is the one-stop-shop for all your behavioral and mental health needs. Through collaborative and attentive services, we aim to provide you with the tools you need to put you on the path to health, wellbeing, recovery and independence.
Our services include:
Individual, group and family counseling or therapy
Supportive and comprehensive care for individuals with serious mental illness
Education, training and other supports that assist you in finding, securing and maintaining employment
Assistance in navigating and connecting to care and other services for people who have chronic illnesses
An on-site pharmacy
We accept Medicaid, Private Insurance, and offer a sliding scale fee. No one will be refused services due to inability to pay or residence. We welcome adults, children and families of all races, ethnicities, religions, gender expressions, and sexual orientations.
