Located in the heart of Midwood, Brooklyn Connected Care is the one-stop-shop for all your behavioral and mental health needs. Through collaborative and attentive services, we aim to provide you with the tools you need to put you on the path to health, wellbeing, recovery and independence.

Our staff comes from diverse cultural backgrounds and includes therapists, nurse practitioners, social workers, community health workers and more.

We can also provide services to those speaking other languages, including ASL, using translation support.

Brooklyn Connected Care accepts most major insurances, including Medicaid, and we offer a sliding scale fee. No one will be refused services due to inability to pay or residence.

The Federal Government's Excellence Act, managed by SAMHSA, has chosen The Jewish Board as a Certified Community Behavioral Health Clinic (CCBHC) expansion site and awarded us with funds to better meet our community's behavioral health needs.

Brooklyn Connected Care is a without walls clinic that combines evidence-based practices, 24-hour crisis services, care coordination, medication-assisted treatment (MAT), peer and family support, and integrated physical health care.

What to Expect When You Call:

  • A receptionist will gather your information and help you schedule an appointment

  • You will be directed to the appropriate program to meet your needs

  • Once accepted, you will complete a comprehensive psychosocial and health assessment
