Services at Brooklyn Connected Care
Brooklyn Connected Care provides all participants with accessible care that is person and family centered. This means that our care is collaborative, respectful and responsive to each person’s unique set of hopes and needs. No one will be refused services based on ability to pay, residence, or documentation status.
Our clinicians speak English, Haitian Creole, Hebrew, Polish, Russian, Spanish, Ukrainian and Yiddish. We can also provide services to those speaking other languages, including ASL, using translation support.

education, training and other supports that assist you in finding, securing and maintaining employment
assistance in navigating and connecting to care and other services for people who have chronic illnesses

Counseling Center
Our licensed outpatient mental health clinic offers individual, group and family therapy for people ages 5 and up.
Care Coordination
Our team addresses the needs of adults and children with chronic illnesses by creating a supportive network to navigate the chaos of multiple providers, locate the resources they need, and develop schedules to stay healthy.
Health Team
Our experienced team of nurses provide screenings and assessments of your physical health, and work with your care team to address your overall health needs.
Pharmacy
In partnership with Genoa Pharmacy, we cover all the medication needs for our community on-site, so they don’t have to travel to get their medications filled.
Additional Services
Family Roots Brooklyn
We work with children and adolescents ages 5 to 20 in managing multiple stressors, mental health conditions, or behavioral issues and can offer our services in the community, home, school, or anywhere else that is most comfortable for them.
Personalized Recovery-Oriented Services (PROS)
We help adults develop their life goals, build healthy relationships, finish their education, find employment, and/or securing housing.
Brighter Pathways
We help you work toward your unique recovery goals, including getting and staying clean and sober, getting a job, or returning to school.
Assertive Community Treatment (ACT)
We offer a full range of services for people with severe mental illness who may be transient and have difficulty connecting to care.
