Our team addresses the needs of adults and children with chronic illnesses by creating a supportive network to navigate the chaos of multiple providers, locate the resources they need, and develop schedules to stay healthy.

 

How we help

 
  1. First, we get to know you by listening to your story. We give you the space to share your history and health needs and offer support in figuring out which needs are being met and which are not. 

  2. Then, we help you take action. Once we understand your specific situation, we work with you to create a care plan that fits your life and guides you through the steps to thrive. 

  3. We build a care team around each person. Has your care ever felt fragmented? Our Care Management team gets all of your health providers on the same page and working towards the health goals that you feel good about. This team may be made up of several different people like a primary care doctor, psychiatrist, therapist, social worker, and other specialists.  

  4. Our Care Coordinators make sure that you’re connected to the resources you need. This might mean matching you with community supports, like Access a Ride or public assistance. Our mission is to help you explore ways to live a healthier lifestyle and meet your health goals, including building positive relationships, eating more nutritious foods, and getting exercise. We make sure all the right people are connected in support of your health. 

Who is eligible?

 

Individuals five years of age and older, with Medicaid coverage, who have a chronic disease including diabetes, asthma, hypertension, heart disease, mental illness, HIV, substance use disorder, or cardio-metabolic disorders.

Brooklyn Connected Care provides services regardless of a person’s ability to pay – this includes the underserved, those with little to no income, those that are insured as well as those who are uninsured or on Medicaid. BKCC also provides care to active-duty military personnel and veterans.
