Our experienced team of nurses provide screenings and assessments of your physical health, and work with your care team to address your overall health needs.

We provide physical health screenings for a variety of health issues. We also offer health education and support you in establishing your recovery goals.   

Our screening services include: 

• HIV screenings 

• Preventative diabetes monitoring 

• Tobacco cessation 

• Weight management

• Substance use screenings 

• Medication-Assisted Treatment (MAT)

• Medication monitoring

• Injectables

• Specimen collection (blood draws/urine)

Anyone age 5 and up.

Brooklyn Connected Care provides services regardless of a person’s ability to pay – this includes the underserved, those with little to no income, those that are insured as well as those who are uninsured or on Medicaid. BKCC also provides care to active-duty military personnel and veterans.
