Our Children Family Treatment and Support Services (CFTSS) program offers community-based services to children and adolescents—ages 5 to 20—and their families.

 

How we help

 

We work with children managing multiple stressors, mental health conditions, or behavioral issues and can offer our services in the community, home, school, or anywhere else that is most comfortable for you. Our work supports the entire family, including parents or caregivers who may be distressed or overwhelmed by their child’s difficulties.   

Services include: 

  • OLP (therapy) 

  • CPST (community psychiatric support and treatment) 

  • Crisis management 

  • In-depth treatment 

  • Solution-focused support 

  • Crisis avoidance 

  • Peer Services

Who is eligible?

 

Families and children 5 to 20 years old who are covered by Medicaid.

Brooklyn Connected Care provides services regardless of a person’s ability to pay – this includes the underserved, those with little to no income, those that are insured as well as those who are uninsured or on Medicaid. Brooklyn Connected Care also provides care to active-duty military personnel and veterans.
